1. Program Overview
Abbott’s Healthy Food Rx program enrolled 284 adults living with diabetes for six months, providing biweekly recipe-based food boxes containing pantry staples, proteins, fruits and vegetables. Participants received health coaching from community health workers in their preferred language, including monthly lessons and twice-monthly follow-ups to support diabetes self-management.
2. Participant Health Improvements
Participants doubled daily vegetable intake from 1.2 to 2.4 servings and nearly doubled fruit intake from 1.2 to 2.3 servings, while water intake rose from 2.8 to 4.1 times per day. Exercise sessions increased from 2.8 to 4.4 times weekly; glucose monitoring climbed from 28% to 62%; medication adherence jumped from 57% to 94%; physical health ratings improved from 21% to 77%; mental health ratings rose from 34% to 88%; and food insecurity fell from 91% to 76%.
