The collaboration integrates care delivery, data and financial infrastructure to improve outcomes and unlock shared-savings revenue opportunities.

1. Collaboration Agreement

The Physician Association of California and Astrana Health have entered a strategic partnership to extend Astrana’s Medicare Accountable Care Organization platform to 15,000 independent primary care physicians. This collaboration aims to help private practices remain autonomous while succeeding in value-based care programs.

2. Platform Capabilities

Astrana’s in-house AI-powered operating system consolidates care delivery, population health management and financial workflows into a single interface. Participating physicians will receive on-the-ground care coordination teams, real-time analytics at the point of care and dedicated operational support to streamline reporting and patient engagement.

3. Financial and Growth Implications

PAC physicians will access enhanced ACO participation pathways with incremental revenue streams, including in-year performance incentives and year-end shared savings. The partnership is expected to bolster Astrana’s ACO enrollment, drive scale across its four 2026 ACOs and expand its shared-savings pool for approximately 75,000 Medicare beneficiaries.
