Centene Foundation and Home State Health granted $225,000 to Three Steps Home to launch St. Louis’ first short-term residential post-acute medical recuperative care program for individuals experiencing homelessness. Located at the Peter & Paul Community Campus, the facility offers clinical care, behavioral health referrals, care plans, meals and housing support.

1. Program Funding

Centene Foundation and Home State Health have committed a $225,000 grant to Three Steps Home to launch St. Louis’ first dedicated short-term residential post-acute medical recuperative care program for individuals experiencing homelessness. The grant supports establishment of a facility capable of bridging gaps between hospital discharge and stable housing for vulnerable populations.

2. Services Offered

The program, located at 3225 N. Florissant Avenue within the Peter & Paul Community Campus, will provide onsite clinical care, behavioral health and substance use referrals, individualized care plans, meals, clothing, laundry, family support and optional spiritual care to promote whole person healing.

3. Community Impact and Opening

This integrated model aims to reduce readmissions and create pathways to stable housing by coordinating nonprofit housing support, healthcare access and benefits assistance. A grand opening and ribbon cutting is scheduled for April 23, 2026, marking the program’s official launch.
