Natera says finalized MolDx coverage expands Prospera surveillance testing for kidney, heart, lung transplants

Natera said a final MolDx local coverage determination (L40060) expands Medicare coverage for Prospera surveillance testing in solid organ transplants.
- The policy takes effect Aug. 30, 2026, and raises allowed surveillance testing frequency across kidney, heart and lung transplant monitoring.
- Kidney coverage is set at 6 tests in year 1, then 4 per year in years 2-3.
- Heart coverage is set at 12 tests in year 1, then 4 per year.
- Lung coverage is set at 12 tests in year 1, then 4 per year in years 2-3 for single or bilateral transplants.
- The final policy exceeds the July 2025 draft, which proposed 2 tests per year in years 2-3 across indications.
