PFE•Pfizer’s Phase 3 trial showed its BRAFTOVI regimen with cetuximab and FOLFIRI nearly doubled median PFS to 15.2 versus 8.3 months in BRAF V600E-mutant metastatic colorectal cancer (HR 0.44; p=0.0002). The regimen delivered a 44% mortality risk reduction (HR 0.56), 72% 18-month survival and secured FDA approval in February 2026.
In the randomized Phase 3 BREAKWATER trial cohort 3, the BRAFTOVI regimen with cetuximab and FOLFIRI achieved a median progression-free survival of 15.2 months versus 8.3 months for FOLFIRI with or without bevacizumab, a 56% reduction in progression or death risk (HR 0.44; p=0.0002). Updated overall survival analysis showed a 44% mortality risk reduction (HR 0.56) with 72% of patients alive at 18 months compared to 54.5% in the comparator arm.
Based on Phase 3 and Cohort 3 data, the U.S. FDA granted full approval in February 2026 for BRAFTOVI in combination with cetuximab and fluorouracil-based chemotherapy as first-line treatment for patients with BRAF V600E-mutant metastatic colorectal cancer, expanding from prior accelerated approval and offering greater regimen flexibility.
The safety profile of the BRAFTOVI combination remained consistent with known effects of each agent; the most common adverse events (≥25%) included nausea, diarrhea, vomiting, anemia and fatigue, while Grade ≥3 events occurred in 70.4% of patients versus 80.9% in the control arm. Treatment discontinuation due to adverse events was 15.5% for the BRAFTOVI group compared to 10.3% for the comparator regimen.
This approval strengthens Pfizer’s oncology portfolio by providing a targeted first-line therapy in a high-unmet-need BRAF V600E-mutant metastatic colorectal cancer segment affecting up to 12% of mCRC patients, positioning Pfizer to capture a significant share of a market with substantial growth potential.
