CVS Health Foundation has committed a $2.24 million investment to establish a Health Zone in Charlotte’s West End through the Westside Wellness Collab. The initiative aims to strengthen community health by expanding access to primary care, healthy food and chronic-condition support in historically underserved neighborhoods.

2. Health Zone Model and Partners

The Health Zone model brings together LISC Charlotte, Care Ring, Charlotte Community Services Association and Deep Roots to deliver co-located services at a central hub and via mobile clinics. Core offerings include preventive screenings, care coordination, nutrition education and fresh food distribution to address multiple barriers in one coordinated effort.

3. Community Impact Goals

Over its three-year duration, the program targets measurable improvements in blood pressure, cholesterol, BMI and reductions in preventable hospitalizations and food insecurity. Success metrics also include increased preventive care visits, enhanced continuity of care and strengthened regional food system resilience.

4. CVS Health’s North Carolina Commitment

This launch builds on CVS Health’s broader North Carolina footprint of over 360 locations and an estimated $4.9 billion economic impact in fiscal 2024.
