DaVita’s CKCC program achieved a 9% increase in Total Quality Score through gains in optimal treatment starts, patient activation and behavioral health support while generating over $200 million in shared savings since inception. Participants accounted for 34% of high-performer awards despite comprising 28% of program members, illustrating efficiency.

In its CKCC demonstration, DaVita achieved a 9% increase in Total Quality Score for 2024, driven by gains in optimal treatment starts, patient activation, and expanded behavioral health support.

2. Shared Savings Achievements

Since program inception, DaVita has delivered more than $200 million in shared savings and accounted for 34% of the CKCC High Performers Pool while comprising just 28% of participants.

3. Patient Outcome Enhancements

CKCC participants showed more planned kidney replacement therapy starts, higher home-treatment adoption, lower central venous catheter use, and fewer missed dialysis sessions, reflecting stronger care coordination.

4. Value-Based Care Scale and Outlook

With over $5 billion in medical costs under management across 3,242 centers serving 295,000 patients by year-end 2025, DaVita plans to leverage these results to expand value-based models in chronic care specialties.
