DaVita partners with Humana on value-based care program for CKD patients
DVA•DaVita and Humana launch CKD value-based care partnership
DaVita entered a value-based care partnership with Humana for Medicare Advantage members with CKD stages 3B-5.
The program launched July 1 and targets earlier intervention to slow progression, delay dialysis, and reduce hospitalizations.
The agreement is expected to cover more than 10,000 patients through DaVita Integrated Kidney Care.
