Ziihera First-Line Regimen Improves PFS 53% and OS 37% in HER2+ Gastroesophageal Cancer
JAZZ•Ziihera combinations improved median PFS to 12.4 versus 8.1 months (HR 0.64) and median OS to 26.4 versus 19.2 months (HR 0.72) over trastuzumab plus chemotherapy in first-line HER2+ gastroesophageal adenocarcinoma. Expanded subgroup analyses confirmed consistent survival benefit across PD-L1 status and regions and OS beyond two years.
1. Phase 3 Efficacy Results
Jazz Pharmaceuticals reported that the Phase 3 HERIZON-GEA-01 trial showed Ziihera plus chemotherapy reduced risk of progression or death by roughly 35% versus trastuzumab combinations (median PFS 12.4 vs 8.1 months; HR ~0.64) and improved median OS to 26.4 vs 19.2 months (HR 0.72).
2. Subgroup and Safety Analyses
Expanded analyses demonstrated that PFS and OS improvements were consistent across PD-L1–positive and PD-L1–negative patients, geographic regions and performance statuses. Safety remained in line with known profiles of zanidatamab, chemotherapy and PD-1 inhibitors, with gastrointestinal events generally time-limited and infrequently causing treatment discontinuation.
3. Guideline Submission and Clinical Implications
Jazz Pharmaceuticals has submitted HERIZON-GEA-01 data to the NCCN for inclusion in first-line HER2+ gastroesophageal adenocarcinoma guidelines. The robust survival benefit and manageable safety could position Ziihera regimens as a new standard of care in this setting.
4. Upcoming Analyses
A second interim overall survival analysis for the Ziihera plus chemotherapy arm is expected in mid-2026 to further assess long-term benefit. Additional prespecified PD-L1 subgroup data will be presented at the 2026 ASCO Annual Meeting to provide deeper insights into efficacy across biomarker-defined populations.
